The medal depicts the Resurrection Gate and a statue of Archangel Michael on the roof of the chapel in front of it. This architectural complex with two towers and arches, a recognizable symbol of the Kremlin, was built in the 18th century and formed part of the Kremlin's defensive complex. The statue of Archangel Michael symbolized the capital's defense against enemies. Archangel Michael is one of the most revered archangels in Orthodoxy, considered the protector and patron of warriors. The gate and chapel complex was an important element of the Kremlin's ceremonial entrance and also served as a venue for ceremonial events.
The medal also contains inscriptions.

The Coat of Arms of the Russian Empire is depicted—a double-headed eagle within an ornamental frame and the mint mark. There is also an inscription listing the Grand Dukes who made the greatest contribution to the construction and development of the Moscow Kremlin.
On the left is a reference to:
Yuri Dolgoruky (1090s–1157)—the founder of Moscow (1156), Prince of the Rostov-Suzdal land, who played a key role in strengthening the northeastern lands of Rus'.
Ivan Kalita (1288–1340)—the Grand Duke of Moscow, who laid the foundations of Moscow's political and economic power and united the Russian lands around the principality.
On the right is a reference to:
Dmitry Donskoy (1350–1389)—the victor of the Battle of Kulikovo (1380), who strengthened Moscow's authority and weakened the power of the Golden Horde.
Ivan III (1440–1505) – Grand Duke of Moscow, “the gatherer of Russian lands”, who finally overthrew the Horde dependence (1480) and laid the foundations of the centralized Russian state.

The central medal weighs 2 troy ounces (62.2 g) and is 50 mm in diameter.
The remaining medals weigh 1 troy ounce (31.1 g) and are 39 mm in diameter.
They are supplied in a wooden case with a certificate.
