Obverse

Bust of the king facing right, in crown and armour, with the Order of the Golden Fleece and in a cloak fastened with a pin on which the date 1614 is inscribed. The year 1614 also appears above the king's crown. On the shoulder the initials SA (Samuel Ammon, medalist). The inscription begins and ends with a decorative floral ornament.

Translation: Sigismund III, by the grace of God, King of Poland, Grand Duke of Lithuania, Russia and Prussia.

Reverse

Coat of arms of Gdańsk supported by two lions. Above the shield is a composition of laurel and palm branches. Below the shield are the separated initials S-A (Samuel Ammon, medalist) and the date 16-14, additionally among the ornaments the initials FB (unknown engraver or medalist).
